Your Treatment Team
A key component of the Integrated Treatment Approach is the Treatment Team. Lead by staff from the Department of Social Services, the Team is comprised of a number of professionals who work with the patient to achieve their treatment goals. The Treatment team includes the Clinical Therapist, Psychiatrist, and Unit Nurse. The team meets daily under the supervision of the Clinical Therapists to review progress of each patient on their Discharge Plan.
Social Services Department
The Social Services Department provides coordination of care while a patient is in the hospital and in preparing for discharge. Key roles in the Social Services Department are:
Clinical Therapists
Clinical Therapists are Master’s level Mental Health professionals. Working within a bio-psychosocial model the therapist utilizes evidence based practice to assess and identify psychosocial, educational, developmental and behavioral functioning of the client and their family system to assist clients achieve their optimal level of mental and emotional health.They provide the lead role in multidisciplinary team meetings, patient care conferences, treatment planning/rounds. They contact the patient’s therapist or case manager in the community, or other treatment provider, get input into the patient’s treatment plan. They formulate and conduct educational, therapeutic and /or support groups, and teach clients the tools they need to participate in treatment. As indicated in the overall assessment, Clinical Therapists provide short term supportive family therapy.
Discharge Planners
The Discharge Planner functions under the direct supervision of the Manager of Social Services. The Discharge Planner assumes responsibility for the overall discharge planning needs and placement of the patient. This includes resident needs, aftercare needs, advocacy needs, and entitlement needs.To facilitate effective discharge planning, the Discharge Planners meet with patients admitted from the previous day and complete discharge planning assessment to determine all discharge planning needs as indicated by treatment team. Review cases with Therapist involved and manage the needs of the patient throughout treatment experience and determine placement outcomes. As part of the Treatment Team, they attend treatment plan meetings and maintain documentation and intervention for treatment planning
The Discharge Planners work actively with each patient assisting patient with phone calls, appointments, and aftercare needs and educate patients on the need for aftercare placement. They communicate with referral sources, health care team members, and community providers regarding client status and progress.
Occupational Therapists
Occupational Therapy Staff provide services that are goal-directed, purposeful activity to aid in the development of adaptive skills and performance capacities by individuals of all ages who have psychological impairments. Such therapy is designed to maximize independence, prevent further disability, and maintain health. They provide functional assessments as part of the patient’s treatment plan. Occupational therapy services include treatment of occupational performance including independent living skills, prevocational/work adjustment, educational skills, play/leisure abilities, and social skills.Recreational Therapists
Recreational Therapists provide an assessment of the patient’s leisure, social and recreational abilities, deficiencies, interests, barriers, life experiences, needs, and potential; treatment services designed to improve social, emotional, cognitive, physical, and functional behaviors as necessary prerequisites to future leisure/social environment; and leisure education designed to help the patient acquire the knowledge, skills, and attitudes needed for an independent healthy lifestyle.Recreational therapy services staff monitor the extent to which goals are achieved relative to the use of leisure time and the acquisition of socialization skills.
Recreational and other leisure time activity services provide for the development, maintenance, and expression of an appropriate leisure/social lifestyle for individuals with physical, mental, emotional, or social limitations.
Students
Fairfax Hospital is a teaching hospital. Therefore, all Health Care Professionals may have students present and they may be involved in your care.Medical Staff
Medical staff oversees the primary patient care while at Fairfax.
Psychatrist
A Psychiatrist works closely with the treatment team in providing quality care for you as he or she directs your medical management. You will be seen a psychiatrist within the first 24 hours after admission. A Fairfax psychiatrist will see you 6 out of the 7 days in a week. You must be seen by your psychiatrist prior to discharge.Registered Nurses
Registered Nurses are responsible for your daily activities and medical/emotional needs. The Nurses supervise the Unit and participate in the management of the environment on the unit. They also administer medications as ordered by the psychiatrists.Nurse Manager
The Nurse Manager assists the Program Manager and the staff in all Unit activities and needs. They are a resource for medication related issues and facilitate communication with the Psychiatrist in regards to medical issues.Pharmacist
Fairfax Hospital has its own on-site pharmacy with licensed pharmacists who work with the Treatment Team to determine the most appropriate medications.Dietitian
A Dietitian is available to discuss any dietary concerns or needs you may have with the Treatment Team.Program Staff
The Program Manager is responsible for managing and directing activities related to patient care in accordance with Standards of Care, mental health practice and Fairfax Hospital policy, goals and objectives.
The Program Specialist assists you with your daily activities. They supervise program groups and activities, including recreation therapy and skill building.

Chemical Dependency Professionals provide education and lead groups focused on chemical dependency issues for mental health, co-occurring disorders and CD patients. They also complete Chemical Dependency assessments.
